Job Summary

Within a multidisciplinary treatment team as part of the Bridge Clinic located at Zuckerberg San Francisco General Hospital, the Clinical Social Worker II (CSW II) will work with people who use drugs and alcohol to provide low threshold/low barrier social work services within a "safety net medical home" ambulatory care setting. These services include, but are not limited to: psychosocial assessments, assisting patients in referrals to emergency and long-term housing stabilization, motivational interviewing, harm reduction counseling, referrals to residential treatment programs for substance use and mental health, mental health assessments and connection to care, and appropriate referrals to community agencies for financial benefits, legal, and immigration resources. Work with treatment team and inpatient/outpatient units to facilitate referrals and service.

The individual will provide a full array of social services that include, but are not limited to: crisis interventions, brief therapy, and provider consultations. The individual may also participate in multi-disciplinary case conferences; in which care plans are developed for high-risk patients. The individual will be required to participate in social work meetings, staff meetings, case conferences, and other required clinic/departmental meetings. The individual may be responsible for providing culturally specific care to monolingual Spanish-speaking patient. The individual may facilitate weekly therapeutic groups. The CSWII will collaborate with substance use navigators and program manager on workflows for linkage to outside programs. The CSWII will have on site clinical supervision by a CSW with extensive experience with this patient population. The CSWII will be a role model for compassionate care for people who use drugs and alcohol.

Department Description

The Department of Family & Community Medicine (DFCM) is engaged in research, teaching, training, service and clinical activities located at seven primary UCSF and leased sites in San Francisco (Parnassus; ZSFG - Buildings 80, 90, 20 and the Main Hospital; Mission Center Building; Laurel Heights; Mission Hall/ Mission Bay; Lakeshore; and Lakeside) as well as the OSHER Center, Women's Health, and the UCSF Primary Care Practices at China Basin, Laurel Village, and at various local community, national and international research/service/education training and clinical sites. In addition to the Department's Family Medicine Residency Program located at ZSFG, the DFCM has affiliated residency programs in Fresno, Salinas, Santa Rosa, Kaiser Vallejo, and Contra Costa Health Systems. Authority and accountability for the Department reside with the Department Chair who delegates authority to Vice Chairs, the Executive Council, Directors and Chiefs of Service and their Administrators and the Associate Chair of Finance and Administration. The mission of the DFCM is to educate students and residents in family medicine with an emphasis on meeting the needs of the economically disadvantaged and the medically underserved, advance knowledge in family and community medicine and develop methods of primary care that are effective, efficient, and accessible to all people. The Adult Urgent Care Center (UCC) is a clinical enterprise of DFCM at ZSFG for patients 18 years and older in San Francisco needing same-day urgent medical care for illnesses or injuries that are not life-threatening. The UCC operates daily (7 days a week, 365 days a year). UCC is open 83 hours per week, and is one of the highest volume clinics at ZSFG with about 26,000 annual visits. The core mission of the UCC is to care for uninsured and low-income adult residents of San Francisco whose urgent care needs are best met by the clinic and provide "the greatest good for the greatest number" of these patients. The UCC provides services to adult patients who are self-referred, referred from San Francisco Health Network (SFHN) clinics, and referred from the ZSFG Emergency Department (ED). The UCC is committed to provide access for all San Francisco residents by eliminating financial, linguistic, physical and operational barriers and provide quality services that treat illness, promote and sustain wellness, and prevent the spread of disease, injury and disability. Authority and accountability for the UCC reside with the UCC Leadership Team (Medical Director, Associate Medical Director, Clinic Operations Program Manager, Department of Public Health [DPH] Nurse Manager, and DPH Administrative Charge Nurse) and the ZSFGH Hospital Leadership.

Required Qualifications

  • Master's degree from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
  • Must be computer-literate, working knowledge of EHR systems, and Microsoft Suite.
  • Demonstrated excellent oral/telephone communication skills and written documentation.
  • Ability to work in a high stress, high volume area and work well under pressure and with deadlines.
  • Ability to handle demanding workload involving multiple tasks effectively and efficiently.
  • Ability to function independently as a collaborative, supportive team member.
  • Must demonstrate cultural humility and a strengths-based, recovery philosophy.
  • Excellent motivational interviewing skills and compassionate approach to people who use drugs, including those who plan to continue using drugs and alcohol
  • Excellent organizational and strong interpersonal skills
  • Demonstrated excellent attendance and reliability

Preferred Qualifications

  • Fluency in Spanish - ability to communicate effectively with monolingual patients
  • Licensed Clinical Social Worker; or Licensed Marriage Family Therapist
  • Two (2) to four (4) years of social work experience providing services to people with substance use disorders that are diverse in gender, sexual orientation, language, country of origin, race, and ethnicity.

License/Certification

  • Possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or equivalent
