What you will do:

  • Complete nutrition assessment for patients who have been identified for at nutrition risk and providing appropriate nutrition intervention for the patients.
  • Work closely with the interdisciplinary team to support optimal nutrition.

What you will need:

  • Bachelor's Degree in Dietetics
  • Registered as a Dietitian with Commission on Dietetic Registration (RD) and Registered Dietitian State Certification or Licensure (CD) in the state(s) of practice.
  • Current Infant-Adult Basic Life Support for Health Care Providers (with hands on learning) from one of the following programs; American Heart Association or American Red Cross.

Job Description:

The Registered Dietitian (RD), Diabetes Care Specialist has expertise in diabetes disease prevention and treatment, and works collaboratively with the physician, associate staff and the RN, Diabetes Care Specialist to provide comprehensive care to patients in the outpatient and inpatient settings. The RD, Diabetes Care Specialist is the expert in individualized Medical Nutrition Therapy and Diabetes Self-Management Education and Support performing assessment, diagnoses, development and implementation of nutrition and diabetes plans. He/she consults with other healthcare providers, arranges referrals, and provides psychosocial support to patients and their families, as needed.

Major Responsibilities:

1. Assesses, diagnoses, develops, and implements Nutrition and Diabetes plans with supervising medical staff to provide high-quality, cost-effective patient care.
a. Uses assessment skills and appropriate risk assessment tools to identify patients with actual or potential health care needs that would require care coordination, therapy, primary care needs, or other referrals.
b. Explores patient's understanding and knowledge of current health status and administers Medical Nutrition Therapy and Diabetes Self-Management Education and Support in a tangible and accessible way.
c. Performs high quality, cost effective, ongoing Medical Nutrition Therapy and Diabetes Self-Management Education and Support in consultation with supervising medical staff to promote optimal health.
d. Instructs patients, families, and caretakers in appropriate diabetes medication self-administration.
e. Assists patients, families, and clinicians with management of diabetes through Diabetes Medication titration.
2. Serves as nutrition content expert and diabetes content expert within multidisciplinary team (internal and external)
a. Consults with of physicians, psychologists, therapists, physician assistants, nurses, case managers, other dietitians, and other members of multidisciplinary teams to provide high-quality, cost-effective referral and consultation for patients.
b. Provides education for departmental colleagues and rotational learners, medical staff, preventative care personnel, and the community to insure a continuum of high-quality care at Gundersen Health System.
c. Participates in organization's diabetes disease management program development and refinement.
3. Promotes patient wellness and facilitates healing.
a. Anticipates future needs and educates or refers to valid sources of information.
b. Understands the patient's unique perspective and assures right education, right time, right environment for learning.
c. Evaluates outcome of learning

4. Delegation and supervision of assistive personnel.
a. Delegates care based on situation.
b. Fosters teamwork and respect.
c. Assures effective use of staffing resources.
d. Supports assistive personnel: Serves as a resource and holds assistive personnel accountable to complete delegated tasks.

a. Continually evaluate the patient's readiness for change and motivation toward goals set using nutrition education, counseling, and motivational interviewing skills to meet agreed upon goals.
8. Performs other job-related duties as assigned.

Education and Learning:

REQUIRED

Bachelor's degree in Dietetics

Master's degree

Work Experience:

REQUIRED

3-4 years of experience as a dietitian or related healthcare field.

DESIRED

1 year of experience in Diabetes Self-Management Education and Support

License and Certifications:

REQUIRED

  • Registered as a Dietitian - Academy of Nutrition and Dietetics

  • Registered Dietitian State Certification or Licenseand some positions may require multiple state licenses

  • Certified Diabetes Care and Education Specialist (CDCES) within five years of hire date

  • Current Infant-Adult Basic Life Support for Health Care Providers (with hands on learning) from one of the following programs; American Heart Association or American Red Cross.
