Overview

The Street Medicine Program Manager provides administrative and operational oversight for the Street Medicine Program and is an integral member of the program's Leadership Team working in close collaboration with the Associate Medical Director, Associate Behavioral Health Director, and Nurse Supervisor. The Program Manager's main duties include supervision of case managers, caseload management, budget management, recruitment, data collection, reporting, quality assurance, and collaboration with program partners and funders. In addition, this position, working in partnership with the Homeless Services Director, provides support for Street Medicine operations across all teams in all contracted Zones including development of workflows, schedules, policies and procedures, coordination of purchasing and equipment, and development and coordination of training for program staff.

This is a full time, exempt, benefit eligible position in Oakland, CA.

Benefits

Responsibilities
    Supervise program case managers including recruitment, hiring, onboarding, evaluation, and disciplinary action
  • Support direct service delivery by conducting outreach and engagement with teams as needed and to support meeting program objectives
  • Lead program and team meetings in collaboration with program Leadership
  • Collaborate with other members of Program and Departmental Leadership in strategic planning and operational implementation of program activities
  • Coordinate program data collection, submit reports to funders and partners, and participate in grant development and management activities
  • Participate in development of IT systems to support optimal program operations
  • Develop and maintain relationships with outside agencies to promote program referrals, collaboration, and access to resources to help people meet their basic needs (e.g. food, water, clothing)
  • Develop and maintain community resources and linkages
  • Ensure client referral and intake process supports contract and funding goals
  • Identify and develop policies, procedures and workflows to ensure contract compliance, create efficiencies, promote quality outcomes, and achieve program goals
  • Coordinate site tours and shadowing opportunities for partners, funders and potential staff in order to promote LifeLong Homeless Services programs
  • Identify training needs and develop and manage staff training opportunities
  • Coordinate and manage purchasing and equipment to support program operations
  • Promote diversity, equity, inclusion, and belonging in the program model and in support of patients and staff
  • Perform other duties as assigned by supervisor
Qualifications

Job Requirements

  • Minimum of 5 years of experience providing community-based support services for people experiencing homelessness or other intersecting populations, especially those with serious mental illness, people who use drugs, or the reentry population
  • Minimum 3 years of experience in program planning, program management, and staff/team supervision
  • Possession of a valid driver's license and insurance
  • Excellent verbal, written, and interpersonal communication skills
  • Excellent skills in Microsoft Office Suite (e.g. Word, Excel, Outlook)
  • Working knowledge of and ability to apply psychodynamic principles from evidence-based practices (e.g. motivational interviewing, problem-solving skills, stress management, critical time intervention, seeking safety, trauma informed care)
  • Demonstrated ability and sensitivity in working with a diverse staff and with a patient population from various ethnic, socioeconomic, education and experiential backgrounds
  • Able to work under pressure and prioritize competing job demands
  • Ability to effectively partner with and manage licensed and non-licensed healthcare staff

Job Preferences

  • RN, MSW/LCSW, MFT/LMFT, MPH, or other related advanced degree/license may be used in place of 3 years of experience as required above
  • Experience providing and managing programs that offer case management services to individuals with multiple challenges including mental illness, addiction, HIV/AIDS, and other health and social issues
  • Knowledge of Alameda County health and social services resources
  • Lived experience relative to people experiencing homelessness (e.g. formerly homeless, social or behavioral health services consumer, criminal justice system involvement, foster care involvement, and/or close family-member of someone with these experiences)
