A key member of the Leadership Team, the Home Care Manager is responsible for the day-to-day leadership, administration, and oversight of operations related to home care and home health services

ESSENTIAL JOB FUNCTIONS:

  • Manages and oversees the daily operations and services provided by the home care team. This includes staffing, care planning, quality of care, participant satisfaction, and program compliance, while ensuring integrated care with the interdisciplinary team.
  • Manages and oversees the delivery of home health services. This includes operationalizing and overseeing compliance with the home health license and coordinating with home health contracted vendors. Works closely with clinic leadership to ensure close communication and effective coordination of medical care.
  • Makes certain the department has adequate staffing, oversees schedules, and adheres to a competency program. Responsible for staff development, training, recruiting, retention, and employee engagement.
  • Monitors and facilitates program compliance with federal, state, local and plan-level requirements.
  • Ensures best clinical practices and that nurses and caregivers are performing within their full scope and providing consistency and continuity of care.
  • Responsible for building highly effective home care department that fully integrates custodial home care and home health services into the overall care plan.
  • Ensures clarity of procedures, standards of care, and collaboration within and between the other departments.
  • Adheres to SEIU 2015 terms and conditions. Participates in collective bargaining, management meetings, or other Union meetings, as needed.
  • Promotes quality of care by participating in Quality Assurance Improvement Plan (QAIP) activities, initiatives, process improvement plans, committees, etc. Provides clinical leadership through oversight of infection control best practices.
  • Monitors and helps develop budget for areas of responsibility.
  • Participates in after-hours rotational on-call duties.
  • Actively participates in and encourages actions that promote good public relations with participants, their families and friends, visitors, and the community.
  • Other duties as assigned.

EDUCATION:

  • Active California Registered Nurse license.
  • Graduate of an accredited college or university with a baccalaureate degree in nursing.

BACKGROUND & EXPERIENCE:

  • Three years of management experience in healthcare setting.
  • At least 1-year experience with frail older adults.
  • Familiarity with Title 22 regulations preferred.
  • Demonstrated skills leading and motivating teams and individuals. Understands the importance of relationship building.
  • Familiarity with the application of Information Technology (IT) to clinical care, including patient care record-keeping, quality assurance, outcomes assessment and pertinent privacy requirements.
  • Ability to work within an interdisciplinary team with initiative, imagination, resourcefulness and flexibility.
  • Interest and ability to work in a multilingual, multicultural environment.
  • Bi-lingual Cantonese a plus.

PHYSICAL REQUIREMENTS:

  • Ability to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
  • Ability to reach, bend and walk.
  • Five finger manipulation.

COMPENSATION
  • Range: $ 128,824.28 - 149,436.16 USD Annual

This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.
