Job Description

InGenesis is seeking an HIV Case Manager to work with our client. In this role, you will provide case management services to individuals diagnosed with HIV to establish and access HIV Care services.

Essential Responsibilities:

  • Follow up on all referrals received from internal staff, outside agencies, and all other sources.
  • Work with other programs to link homeless individuals diagnosed with HIV to medical care.
  • Collaborate with local correctional agencies and assist individuals involved in the criminal justice system living with HIV with medical care.
  • Verify program eligibility, develop a plan for individuals to enter HIV Primary Care and other HIV Care Services, and ensure follow-up until care is solidly established.
  • Accompany clients to appointments at other agencies as needed.
  • Deliver services on a mobile basis, meeting clients in the field as needed.
  • Works a flexible schedule, including evenings and weekends and overtime, as necessary.
  • Attend workshops, training, and meetings as needed, and as requested.
  • Perform other duties as assigned by the supervisor.

Minimum Qualifications:

  • Bachelor's degree in healthcare, social welfare, psychology, public health, or a related field or an equivalent combination of education and experience.
  • Prior work-related experience with HIV+ populations, active substance users, homeless individuals, transgender people, MSM, and other vulnerable groups.
  • Access to reliable transportation.
  • Proficient with PC-based applications - Word, Excel, and Outlook.

Preferred Qualifications:

  • Minimum of two years of experience providing HIV services related to this position, including experience working with homeless and formerly incarcerated people.
  • Bilingual in Spanish
  • Demographic/geographic knowledge of Santa Clara County.

Work Conditions:

Travel locally to visit other sites, may require a day or overnight trip to attend events; the workload may be demanding at times; and require professionalism and confidentiality.

Physical Demands:

This position requires:

  • Sitting, walking, and standing associated with working in a normal medical/clinical office environment and with providing services out in the field.
  • Regular bending and stretching; and lifting.
  • Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
  • Traveling to work sites includes some exposure to outside elements when in transit and when meeting with clients out in the field.
  • Presenting information in front of a group.
  • Reading and writing in order to review records for accuracy and maintain client files.
  • Occasional evening or weekend work may be necessary.
  • Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
  • Will work with blood or blood-borne pathogens and will require OSHA training.

This position is dependent on contract award.
