POSITION: Social Services Case Worker (DN9S5730)

DEPARTMENT: (100 Corps Community Center) / Hayward Corps.

LOCATION: Hayward, CA

SUPERVISOR: Corps Officer(s), Major Dan Wilson

STATUS: Non-Exempt / Part-time / Temporary April through October 2023

BASIC PURPOSE

The purpose of this position is to assist clients of the Hayward Corps Social Services/Food Pantry. It is also to assist clients in Pathway to Hope to set goals and help develop steps to reach those goals in ways of: referrals, envisioning, training, follow-up sessions, and to achieve a level of self-sufficiency by reaching those goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Adheres to agency's policies and procedures.
  2. Oversees the Food Pantry, Food Pantry operations and Food Pantry volunteers.
  3. Oversees USDA Distribution and seasonal food distribution.
  4. Organizes, maintains and reports all statistical, GIK and other records to proper departments of The Salvation Army, USDA and Alameda Food Bank.
  5. Coordinates with Kitchen Food Service personnel on providing, maintaining, and improving food service to clients.
  6. Provides holistic case management to clients within the Pathway to Hope Program. Case management tasks will include intake, assessment, information and referral, housing case management, application assistance, placement, and follow-up services.
  7. Provide case management geared to areas such as housing assistance and job development. Employment case management services may include referrals to: job preparation, job search, and workshops. Housing case management may include intake, assessment, information and referral, including application assistance, placement, and follow-up services.
  8. Implements case management standards as required by all funding sources including The Salvation Army
  9. Establishes and maintains a housing referral system for clients, including landlords, rental agencies, management companies, local and regional housing authorities.
  10. Assists in coordinating periodic workshops for clients and potential clients when needed.
  11. Maintains accurate and confidential case records on each client served.
  12. Enters, tracks and reviews Bridges/Well-Sky client data.
  13. Ensures Pathway to Hope case management sessions are available to each client at least monthly. Pathway to Hope volunteers may be trained and utilized.
  14. Attends trainings as required by The Salvation Army.
  15. Completes and submits all required paperwork in a timely fashion as designated by the supervisor. Meets deadlines and department productivity demands.
  16. Coordinates Pathway to Hope with the Corps Officers to ensure rules are being followed, proper accountability is being enforced/maintained.
  17. Works with Corps Officer to assist with monitoring the front desk/area.
  18. Acts as role model within and outside the agency.
  19. Maintains a positive respectful attitude.
  20. Communicates regularly with Corps Officer about program issues.
  21. Consistently reports to work on time prepared to perform duties of position.

CERTIFICATES, LICENSES, REGISTRATIONS

  1. Must possess a valid California Class C Driver License, and ability to drive a Salvation Army vehicle
  2. Must be 21 years or older.
  3. Authorize The Salvation Army to add name to the CA DMV Pull Notice Program
  4. Complete The Salvation Army vehicle course training.

PHYSICAL REQUIREMENTS:

  1. Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
  2. Ability to grasp, push, and/or pull objects
  3. Ability to reach overhead
  4. Ability to operate telephone
  5. Ability to lift up to 25 lbs. for administrative positions
  6. Ability to operate a computer
  7. Ability to process written, visual, and/or verbal information
  8. Ability to operate basic office equipment and tools PC, Fax Machine, Telephone, Calculator, Copier, Printer.
