Overview

The Clinical Case Manager (CCM) works with patients and partner organizations to provide short-term intensive transitional case management until a patient has stabilized enough to transition to a conventional system of care (primary care and/or a community mental health clinic or Intensive Case Management program) or has been placed in a higher level of care. As a key member of a multidisciplinary team, the CCM is the lead for enhanced case management (ECM), outreach and engagement to clients with severe behavioral health care symptoms, care plan development, intensive short term case management of a 25-patient caseload, collaboration with licensed mental health clinicians, and the larger medical and mental health system. The CCM is also the team lead to conducting housing assessments and supporting patients to secure housing when matched to a housing resource.

Ideally, the CCM will be a trusted member of the community in which they will work. This relationship will enable the CCM to serve as a liaison between health, social services, and housing resource providers. Following LifeLong COVID-19 safety protocols and exercising good judgment regarding risk mitigation are important for both staff and patient safety.

This is a grant funded, full time, benefit eligible position.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Responsibilities
    Participates with street health team members in conducting street outreach, intake and assessment for people living in homeless encampments.
  • Creates client centered care plans and implements brief interventions and short-term support using appropriate modalities for patients with depression, anxiety, trauma, grief/loss, addiction etc.
  • Supports linkages and coordinated care with public and community based mental health and substance use treatment systems.
  • Provides intensive case management to a caseload of 25 clients, focused on linkage to health care, housing and benefits.
  • Assists with crisis intervention as needed and including assessment, de-escalation, and notifying the public mental health crisis response team if necessary. Follow up to crisis intervention as appropriate.
  • Participates in daily huddles, outreach and follow up planning and weekly case conference ensuring coordination of care with medical providers and other inter-disciplinary team members.
  • Maintains clinical and administrative records in accordance with contractual and billing requirements.
  • Consults with medical providers as needed.
  • Follows all State of California mandated reporting requirements.
Qualifications
  • Ability to work flexibly and collaboratively in both non-traditional, street and encampment settings and in a fast-paced primary care clinic.
  • Must be an independent self-starter, able to balance outreach, counseling, case management and administrative tasks.
  • Ability to work effectively with a multi-disciplinary team.
  • Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle, and experiential backgrounds.
  • Excellent verbal, written and interpersonal communication skills.
  • Computer literate in MS Office applications and willing to learn clinic systems.

Job Requirements

  • Minimum of 2 years of experience working with homeless or other high-risk populations.
  • A Master's degree in social work and registration as an Associate Social Worker with the California Board of Behavioral Sciences can be substituted for the 2 years of experience requirement.
  • Working knowledge of the local behavioral health service system.
  • Familiarity with evidence-based practices for behavioral health disorders.
  • Experience in clinical case management and harm reduction.
  • Possession of current driver's license and insurance.

Job Preferences

  • Lived experience relative to working with people experiencing homelessness (e.g. formerly homeless, social or behavioral health services consumer, criminal justice system involvement, foster care involvement, close family-member of someone with these experiences).
